The Integrated Health Care Management Program (IHCM) provides low-barrier intensive case management. IHCM works specifically with high-needs participants who face challenges accessing medical and behavioral health services for multiple diagnoses. The team supports participants in meeting their self-identified goals by increasing their engagement in health services through care coordination and advocacy.
Monday – Thursday, 9 a.m.–4 p.m.
Friday, 10 a.m.–4 p.m.
